Home health care is an array of medical services ordered by your physician and provided to you in your place-of-residence.

Home health care can be short term or long term.

When is home health care necessary?

The following list may help you identify when home health services may be necessary for your recovery and rehabilitation at home:

• Following a stay at a hospital or rehabilitation center
• Diagnosed with a new illness or disease
• Significant change in your medical condition or status
• Change in your medications
• Recent fall and increased risk of falls
• Chronic disease requiring clinical management such as CHF, COPD, Alzheimer's, MS, or Diabetes
• Supplemental skilled care for dialysis services or wound treatments
• When seeking palliative care
